OnPoint Family Medicine at Westminster is searching for a Nurse Practitioner or Physician Assistant to join the team!

About the Role:

The Nurse Practitioner or Physician Assistant in Westminster will play a pivotal role in delivering high-quality outpatient care to a diverse patient population. This position focuses on comprehensive patient assessment, diagnosis, and management of acute and chronic conditions within an outpatient setting. The successful candidate will collaborate closely with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans that promote patient health and well-being. They will also be responsible for educating patients and their families on disease prevention, treatment options, and health maintenance strategies. Ultimately, this role aims to enhance patient outcomes through compassionate, evidence-based care and effective care coordination.

Responsibilities:

  • Conduct thorough patient histories and physical examinations to assess health status and identify medical conditions.
  • Diagnose and manage acute and chronic illnesses, prescribing medications and therapies as appropriate within scope of practice.
  • Develop and implement individualized treatment plans, including follow-up care and referrals to specialists when necessary.
  • Provide patient education on disease prevention, medication adherence, and lifestyle modifications to support long-term health.
  • Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimize patient outcomes.
  • Maintain accurate and detailed medical records in compliance with regulatory standards and organizational policies.
  • Participate in quality improvement initiatives and stay current with clinical guidelines and best practices.

Minimum Qualifications:

  • Current and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of practice.
  • Current DEA Registration
  • For NP - Prescriptive Authority
  • Master’s degree from an accredited NP or PA program.
  • Certification by a nationally recognized certifying body (e.g., AANP, ANCC, NCCPA).
  • Demonstrated experience in outpatient care management, including patient assessment and treatment planning.
  • Basic Life Support (BLS) certification and must maintain throughout employment.

Preferred Qualifications:

  • Experience working in a primary care or specialty outpatient clinic setting.
  • Familiarity with electronic health record (EHR) systems and telehealth platforms.
  • Advanced Cardiac Life Support (ACLS) certification.
  • Additional training or certification in chronic disease management or preventive care.
  • Strong communication skills with a focus on patient-centered care.

Skills:

The required skill of outpatient care management is essential for effectively evaluating and treating patients in a non-hospital setting, ensuring timely and appropriate interventions. Candidates will use clinical reasoning and diagnostic skills daily to manage a wide range of health conditions, coordinating care with other healthcare providers. Preferred skills such as proficiency with electronic health records and telehealth technologies enhance efficiency and patient access to care. Strong communication and patient education skills are vital for fostering trust and encouraging adherence to treatment plans. Additionally, skills in quality improvement and evidence-based practice support continuous enhancement of care delivery and patient outcomes.

Salary: $110,000 - $140,000
